2. Verification of Physical/Health-Related Disabilities form You should complete the first page of this form; pages 2-4 need to be completed by your health-care provider.
3. Academic Accommodations Request form. A separate form needs to be filed for each course in which you would like assistance.
4. If you would like us to notify your instructors of your disability and any special accommodations, please print and sign our Letters to Instructors form.
